What ages of children are seen at Sulzbacher Village Pediatrics?
We offer services to children from 0-18 years of age.
What services do you offer?
We provide care in our patient-centered medical home which means most of your child’s health care needs can be met in one setting. Services include medical, dental, vision and behavioral health care. Should your child need a specialist, we can arrange that as well.
Are pediatric health services only available to those who are homeless?
No; our pediatric health services are available to all children and their families in Jacksonville regardless of income or housing.
How do I make an appointment for my child?
For all NEW patients, please call 904.394.4958 or request an appointment here. You may complete new patient registration materials ahead of time or in our office.

We offer telehealth appointments for existing behavioral health care patients.
What is the process and timeline to become a new patient?
Once you’ve completed our new patient registration paperwork, we typically can see your child within 7-10 days.
What kind of insurance do you accept?
We accept most public and private insurance plans. No one will be denied care based on your ability to pay as our services are offered on a sliding fee scale.
What are your office hours?
Appointments are available Monday through Thursday from 8 a.m. to 5 p.m. and Friday from 8 a.m. to 3 p.m.

Our pediatric health center is located on the first floor of the Sulzbacher Village at 5455 Springfield Blvd. in the Brentwood neighborhood of Jacksonville.
