This Privacy Policy provides information about how Sulzbacher (“Sulzbacher”, “we”, “us”, “our”) may collect, use, share, or otherwise process personal information obtained through sulzbacherjax.org and/or villagepeds.org (the “Website”), or via our iOS and Android mobile applications (the “Portal”). The Websites and the Portal are collectively referred to in this Online Privacy Policy as the “Sites”.
Sulzbacher also maintains a separate notice, our Notice of Privacy Practices, that applies to protected health information that we collect from individuals pursuant to the Health Insurance Portability and Accountability Act (HIPAA). Click here to view our Notice of Privacy Practices.

Collection of Personal Information
Personal Information You Provide via the Website. We may obtain personal information, which is information that can be used to identify you individually, from you directly when you interact with the Website (for example, when you donate to Sulzbacher). The personal information you might provide to us in this context includes your name and contact information, such as postal address, email address, and phone number.
Personal Information You Provide via the Patient Portal. We may collect certain patient information via the Portal. For more information on these data collection practices, please view our Notice of Privacy Practices.
Information Collected Automatically from the Sites. We, and the vendors we work with, may use various tools and technologies like cookies and pixels to collect certain information automatically about you when you visit our Sites. This information may include IP addresses, pages visited, searches performed, device types, and browser information.

Cookies may allow a website to recognize a user’s device, record user preferences, track the pages a user visits on the website, and identify the referral website. Information Collected Automatically via the Website
We may use third-party technology to collect and use data in connection with your activity on the Website, including for interest-based advertising purposes. For example, we’ve implemented Google Analytics Advertising features such as dynamic remarketing, interest-based advertising, audience targeting, behavioral reporting, demographics and interests reporting, user segment analysis, device reporting, display advertising, and video ads reporting. These ad technology companies and advertisers may use, store, or access cookies and other tracking technologies to collect or receive information from the Website, and elsewhere on the internet and use that information to create a profile of you, measure your interests, detect your demographics, detect your location, detect your device, personalize content, and provide advertising services to us, including advertisements targeted to your interests.

We may use third-party analytics tools such as Mouseflow to track visitor click/touch, movement, scroll, and keystroke activity on the Websites and Scheduler. To opt out of Mouseflow, visit: mouseflow.com/opt-out/.
Information Collected From Other Parties. We may combine information that we collect about you on our Website, including information collected automatically, with information we obtain from others. To the extent we combine information that we collect automatically or that we obtain from others with personal information you provide to us, we will treat the combined information as personal information in accordance with this Privacy Policy. When you submit information to another party, you are solely subject to that party’s terms of use and privacy policies, for which we have no responsibility.
Mobile App Permissions
If you access the Portal via a mobile device, the Portal may, depending on your device and device settings, have access to the following:
- Location (for example, may include precise or approximate location in order to map the nearest pharmacy to you)
- Storage (the Portal may need access to the stored files on your mobile device for you to upload such files through the Portal)
- Camera (may allow the Portal to use your device’s camera for telehealth functionality)
- Microphone (may allow the Portal to use your device’s microphone for telehealth functionality)
- Other information (for example, the Portal may be able to provide push notifications from or to detect a device’s motion to determine if a telehealth user is in a moving vehicle)
By changing your mobile device permission settings, you may affect your ability to access or use certain features on the Portal.
Additionally, you may be able to log into the Portal using your device’s login features such as using a passcode or your fingerprint. These device login features are enabled through your device, not Sulzbacher’s, and are subject to your device settings and preferences.
How We Use the Information We Collect
We may use personal information we collect through for various purposes including:
- Providing our services;
- Facilitating and personalizing your user experience, including by improving our digital properties;
- Sending marketing communications to you that we believe may interest you;
- Monitoring and analyzing trends, usage, and activities of visitors and users;
- Preventing, investigating, identifying, or taking any other action with regard to any suspected or actual fraudulent or illegal activity or activity that violates our policies;
- Complying with applicable laws, rules, regulations, and legal processes as well as our company policies; and
- For any other purpose, with your consent.
We will store information collected via our Sites for no longer than is necessary for the performance of our obligations or to achieve the purposes for which the information was collected, or as may be permitted under applicable law.
Sharing Personal Information
We may share personal information we have obtained as described above with our affiliates, business partners, and vendors. For example, we may use service providers to help us operationally or to administer activities on our behalf, such as sending emails.To the extent we grant third parties access to information collected via your mobile device, they are expected to follow privacy practices no less protective than our practices to the extent allowed by applicable law. In addition, we may share personal information: (1) as permitted or required by federal and state laws and regulations; (2) in the event we sell or transfer all or a portion of our business assets (e.g., further to a merger, liquidation, reorganization, or any other business transaction, including negotiations of such transactions); (3) when we determine that disclosure is necessary or appropriate to comply with law or legal process or to protect legal rights, property, or safety; (4) to enforce our policies; or (5) with your consent or at your direction.

Linked Third-Party Online Platforms
Sulzbacher provides several hyperlinks that lead to other online platforms outside of the Sites, including social networks operated by unaffiliated parties. This Privacy Policy applies only to the Sites. Please note that if you visit a third-party online platform that is not operated by Sulzbacher, that third party may collect information about you. We are not responsible for the privacy practices of other websites or other online platforms. You should review the terms, policies, and settings of these online platforms to learn more about their data practices.
Security
We take reasonable measures to protect the personal information we obtain from loss, theft, misuse, and unauthorized access, disclosure, alteration, and destruction. Nevertheless, transmission via the Internet and online digital storage are not completely secure, so we cannot guarantee the security of your information collected through our Sites.
Your Choices
Communication Preferences and Marketing. If you do not wish to receive marketing emails from us, you can opt out of future emails by following the instructions provided in our emails. Opting out of marketing emails will not affect our administrative emails to you.
Mobile App. You can stop all future collection of data generated by use of the Portal through the mobile app by uninstalling the mobile app. Also, you may be able to exercise specific privacy choices, such as enabling or disabling certain features (e.g., location-based services or push notifications), by adjusting the permissions on your mobile device. Beware that even if GPS precise location services are disabled, other means of establishing or estimating location (e.g., proximity to Wi-Fi or Bluetooth signals) may persist.
Note that uninstalling the mobile app or changing your mobile app permissions for the Portal will not delete your user account or any information we previously collected via the Portal.

Information for Users Outside the United States
Please be aware that any personal information you submit will be transferred to and processed in the United States and the data protection laws and regulations that apply to your personal information may be different from the laws in your country of residence.
Children’s Privacy
We do not knowingly collect personal information online from individuals under the age of 13. If we become aware that we have obtained personal information from an individual under the age of 13, we will delete such information in accordance with applicable law.
Revisions to this Privacy Policy
This Privacy Policy is subject to change at our discretion. We will indicate changes to the Privacy Policy by updating the “Effective Date” at the beginning of the Privacy Policy. Your continued use of the Sites after any update to this Privacy Policy will constitute your acceptance of our changes.
